Assumption of Risk, Waiver of Liability & Release, Indemnification, and Medical Treatment Authorization
I acknowledge that youth soccer involves inherent risks of injury, including but not limited to sprains, fractures, collisions with other players, and — in rare cases — serious or catastrophic injury. I voluntarily allow the player named on this form to participate in Real State Soccer Academy training, practices, tryouts, scrimmages, games, tournaments, and related travel, with full knowledge of these risks.
In consideration of being permitted to participate, I, on behalf of myself, the player, and our heirs and legal representatives, release, waive, discharge, and covenant not to sue Real State Soccer Academy and its owners, coaches, staff, volunteers, and the owners/operators of any facility used for training or competition, from any and all liability, claims, demands, or causes of action arising out of ordinary negligence, for any injury, illness, or loss the player may sustain while participating — to the fullest extent permitted by the law of the jurisdiction in which the applicable program operates. This release does not apply to injury caused by gross negligence or willful misconduct, where such a limitation is not permitted by law.
I authorize Real State Soccer Academy staff to obtain emergency medical treatment for the player if I cannot be reached immediately, and I agree to be financially responsible for any costs of that treatment not covered by our own insurance. I confirm the medical information provided on this form is accurate to the best of my knowledge.
I also grant Real State Soccer Academy permission to use photos or video of the player taken during club activities for the club's own website, social media, and promotional materials, without compensation.
I confirm I am the parent or legal guardian of the player named on this form, I am at least 18 years old, and I have read and understood this waiver in full before agreeing below.
